These are your Entrust Financial Credit Union Teen Card Terms and Conditions. Please read the following carefully and keep these Terms and Conditions for your records as they set out the Agreement between you and us. You will be deemed to have accepted the Card and the terms of this Agreement if you do any of the following: (a) use the Card or (b) activate the Card.
"Agreement" means these Entrust Financial Credit Union Teen Card Terms and Conditions. "We" "us" and "our" refer to Entrust Financial Credit Union. "You", "your", "yours", and "cardholder" in this Agreement refer to the Parent, Teen, or anyone authorized to use or accept an Entrust Financial CU Teen Card. "Card" means the primary Entrust Financial CU Teen Card and any secondary Teen Cards obtained from us. "Parent" means either the parent or legal guardian of the Teen. "Teen" means a child of the Parent who is of at least 13 years of age, who is authorized by the Parent to use the Card. By authorizing the Teen to use the Card, the Parent represents that such Teen is at least 13 years of age. Any use of the Card by anyone under 13 years of age is an unauthorized use of the Card. You, not Entrust Financial Credit Union, have sole responsibility in such cases.
Important Information about Opening Your New Account
To help fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify and record information that identifies each person who opens an account.
The Card is a Visa prepaid, reloadable, stored value card. The Card is not directly linked to any other checking or credit card account. The Card is not a credit card or a NCUA insured deposit account. No interest will be paid on the balances transferred to the Card account. You and the Teen must be residents of the United States to be eligible to have a Card issued and monies funded into the account accessed by the Card. As part of the account set up process, you may establish a designated funding account (the "Funding Account") from which funds will be transferred as authorized to the Card. The Funding Account must be either an Entrust Financial CU Credit Card or Debit Card. You may change the Funding Account from time to time. Each primary cardholder may have a maximum of two secondary cards per an account. Secondary cards will have to be separately activated and can be used in the same way as the Card at ATMs and merchants that accept VISA debit cards. You are responsible for all authorized transactions made with this additional Card(s).
Ownership of the Card
This card is issued by Entrust Financial Credit Union; pursuant to a license from Visa U.S.A, Inc. Your Card allows you to access your prepaid funds at ATMs, which display the Visa or Plus logo. You may also use your Card at merchants who accept VISA debit cards worldwide. The Card is and will remain our property. However, you will be solely and completely responsible for the possession, use and control of the Card. You must surrender the Card to us immediately upon request. The Card may not be used for Internet lottery, betting, or gambling transactions or for any illegal transactions. You agree to take steps to ensure that you do not use your Card or any linked account for any transaction that is illegal under the laws governing your account. Entrust Financial Credit Union reserves the right to deny transactions or authorizations from merchants apparently engaging in the Internet gambling business or identifying themselves through the Card transactions record or otherwise as engaged in such business.
Before using the Card
You must: (i) activate the Card and (ii) sign the reverse of the Card using your usual signature immediately upon receipt. Write down your Card number and the Member Service Number on a separate piece of paper in case the Card is ever lost or stolen. At the time of activation, for your protection and for regulatory purposes, we may request, and you agree to provide, some additional identification verification information such as your home phone number, date of birth, social security number and zip code. We may use this data for a range of purposes, including but not limited to facilitating refunds if your Card is lost or stolen, enhancing usage at merchants that may require zip code authorization, and aiding in collection efforts in the event of a Shortage, as defined herein. If you do not wish to provide the data, or if the data you provide us cannot be appropriately verified, we reserve the right to cancel your Card and reimburse you for the Initial Load Amount.
Using Your Card
You are able to use your Card to:
1. Pay for purchases of goods or services at merchants who accept Visa® debit cards, and have agreed to accept the Card for Point-of-Sale (POS) purchases.
2. Obtain cash at Financial Institutions that accept Visa debit cards.
3. Perform transactions listed below on automated teller machines (ATM) that display the Visa or Plus logo.
4. Access our on-line services to obtain Card account balances and review recent Card account history.
5. Access our telephone customer services number listed below.
Use of Card, the PIN and Terminal The cardholder may be provided with a Personal Identification Number (PIN), which will allow them to use the Card at an ATM. The cardholder can activate the Card and obtain a PIN by calling toll-free, 1-866-760-3156 or visit us online at our website, www.entrustfcu.org. The Card and PIN are provided for your use and protection, and you agree:
1. to not disclose the PIN nor record it on the Card or otherwise make it available to anyone else;
2. to use the Card, the PIN and any ATM or point of sale terminal as instructed;
3. to promptly notify us of any loss or theft of the Card or PIN; and
4. to be liable for any transactions made by a person you authorize or permit to use the Card and/or PIN. If you permit someone else to use your Card, we will treat this as if you have authorized this person to use your Card and you will be responsible for any transactions initiated by such person with your Card.
You may load value onto the Card at any time using a Funding Account, not to exceed $6,000 per seven (7) day period. Upon authorization, this will cause monies to be funded into the account accessed by the Card and be available for use by you to make designated transactions. Funds may not be available for immediate use after you have initiated the transfer. Only the primary cardholder can load funds to the account. You can add value at Entrust Financial Credit Union's website, www.entrustfcu.org, or by calling toll free 1-866-760-3156.
The maximum Card value is $2,000.00. If the Card has $2,000.00 available for purchases or cash withdrawals, additional amounts may not be loaded onto the Card. The maximum amount that can be transferred to the Card at any one time is $2,000.00.
The minimum value that may be loaded onto the Card is $25.00.
The maximum amount allowable for cash withdrawals from ATMs is limited to three (3) ATM withdrawals per day with a maximum of $200.00 per day. The maximum amount allowable for cash advances from Financial Institutions that accept Visa debit cards is limited to three (3) cash advances per day with a maximum of $200.00 per day. The maximum amount allowable for cash back from POS transactions is $200.00 per day, not to exceed three (3) cash back POS transactions per day.
When you use the Card to obtain goods or services or to obtain cash, the merchant may attempt to obtain preauthorization from the Card account for the transaction. A five (5) day hold may be placed on the Card account for the amount of the preauthorization request. If the preauthorization request amount varies from the actual transaction posting amount, the preauthorization hold amount may remain on the account until the five (5) days expire. As a result, subsequent preauthorization requests against the hold amount may cause the transaction to be declined.
When you use your Card at an automated gas station pump you will be required to have a minimum of $75.00 available credit on the Card to enable gas to be dispensed. If your available balance is less than this, please pay the attendant in the gas station.
If you use the Card to make a telephone call this may require of minimum credit of $50.00.
If you use your Card to purchase meals or drinks at a restaurant or bar, a tip of 20% is factored into the amount when pre-authorizing these Card purchases. You will only be debited with the actual amount you spend. Hotels, auto rentals and cruise liners operated on a similar policy, at a rate of 15%.
Should you use your Card to pre-authorize a hotel reservation any amount requested in a pre-authorization will not be available to you to spend using the Card, although you will only be debited with the actual amount you spend.
When making a purchase on the phone, by mail or online the account information you provide must be the same account information as registered for the Card and any difference may lead to a declined authorization.
If you use the Card to buy goods and you want to use some other method of payment for part of the cost please advise the store in advance and use the Card first before paying any balance by other means. We do not guarantee that the establishment will fulfill this request.
Your Card will expire on the date embossed on the front of the Card, which will be not less than twenty-four (24) months after you purchased the Card. If your account is in good standing, has had activity in the last twelve (12) months and has enough of a balance to pay the reissue fee, the Card will automatically reissue to the address on our system.
Each time you use the Card the amount of the transaction will be debited from the account associated with the Card. You are not allowed to exceed the funded balance available on the Card by any individual or series of purchases. Nevertheless, if you make a purchase which exceeds the balance on the Card (an "overdraft"), you shall remain fully responsible for either the amount of the purchases approved or the cash withdrawn which exceeded the balance available on the Card. We also reserve the right to automatically debit such overdrafts from current or future transfers of funds to the Card, or any other account you maintain at Entrust Financial Credit Union.
You agree to immediately forward payment to us for all overdrafts. If the balance on the Card account remains at a negative balance for fifteen (15) days, you agree that we may initiate a transfer from any account you maintain at Entrust Financial Credit Union for the amount of such negative balance.
You agree that the following fees and service charges apply to the Card account and may be charged to the Card account or Funding Account (as specified below for each fee). You authorize us to initiate any such charges to the Funding Account as applicable.
1. The One-time Primary Card Purchase Fee is $8.95 and will be assessed to the Funding Account at the time the Card is opened.
2. A Secondary Card Purchase Fee of $8.95 will be assessed to the Card account for each Secondary Card that is purchased.
3. A charge of $5.00 will be assessed to the Card account if we issue a new Card due to card expiration, replacement or lost/stolen.
4. An Express Delivery Fee of $25.00 will be assessed to the Funding Account for delivery of a new Card or to the Card account for any replacement Cards using a method other than standard mail.
5. A $3.00 fee will be charged to the Funding Account for each transaction that loads money onto the Card. This fee will be waived when direct deposit is used to load funds into the Card account.
6. A charge for ATM cash access of $2.00 will be assessed to the Card account for each transaction where by you obtain cash from the Card account at Visa or Plus ATMs. This fee is in addition to the fee charged by the ATM owner. The ATM owner must disclose any fee(s) they charge prior to you confirming the transaction.
7. A charge of $2.00 will be assessed to the Card account for each cash advance taken against the Card at a Financial Institution that accepts Visa debit cards.
8. A charge of $2.00 will be assessed to the Funding Account for each non-electronic (paper) copy of the monthly Card statement.
9. A charge of $5.00 per month will be assessed to the Card account after twelve (12) months of inactivity.
10. Unclaimed Funds. The balance remaining on the Card may become unclaimed funds escheatable to the State, if, as shown by the Credit Union's records, you have not, within the applicable statutory period, requested refund of unused funds on your expired or canceled Card, corresponded with the Credit Union concerning the Card, or transacted any business on the Card. If that occurs, the funds will escheat to the State in which your last know address was located according to Credit Union records or the State where the Card was purchased.
11. A Foreign Transaction Fee equal to 2.50% of the transaction will be charged to the Card account at the time a foreign purchase/withdrawal is made.
Verification of Transactions
You are entitled to receive a transaction record or receipt each time a transaction, greater than $25.00, is completed using the Card at an electronic terminal or ATM. In addition, all Card transactions will be listed separately on the online Card account statement.
You agree that we may provide your monthly statement and any other notices hereunder electronically. Statements provided electronically will describe each transaction made using the Card during the statement period. Your statement will be available to you in electronic format for viewing. You may print your statement for your record keeping or future reference. You may review the past twelve months worth of transactions online at any time. In the event that you elect to revoke consent to receive statements electronically a fee will apply and we reserve the right to terminate the Card account.
You may review the last five (5) Card transactions through our telephone member service number 1-866-760-3156.
Failure to Complete Transactions
We accept no liability to complete any transaction for which adequate funds have not been supplied by you according to the rules of the Card.
Likewise, we accept no responsibility for funding delays that are the result of late receipt of funding payments by you to the Card account.
We are not responsible if you do not have enough money in the Card account to complete a particular transaction. You may, however, split the purchases between the Card and another credit or debit card or cash. If you are uncertain as to your exact balance available on the Card, please review your Card account online, or call Entrust Financial CU member service to verify the Card's available balance prior to attempting to make a purchase.
Neither we nor any other bank or business will be liable to you for the failure to accept or honor the Card.
Disputes/Returns and Refunds
You agree to settle all disputes about purchases made using the Card with the merchant who honored the Card.
If you are entitled to a refund for any reason for goods or services obtained with the Card, you agree to accept credits to your Card account in place of cash.
International Transactions Foreign Currency Exchange Rates - If you use the card for an international transaction in foreign currency, Visa International will convert the transaction amount from the foreign currency amount to a United States Dollar amount, in accordance with its currency conversion procedures set forth in its Operating Regulations in effect at the time the transaction is processed. Currently those regulations provide that the currency exchange rate is either (1) a rate selected by Visa from the range of rates available in wholesale currency markets for the applicable central processing dates, which rate may vary from the rate Visa itself receives, or (2) a government-mandated rate in effect for the applicable central processing date, plus a Currency Conversion Fee as discussed below. The currency exchange rate in effect on the processing date may differ from the rate in effect on the transaction date or on the posting date.
Foreign Transaction Fee - A Foreign Transaction fee is charged for each international transaction (i.e., a transaction occurring in any country outside the United States, Puerto Rico or the U.S. Virgin Islands), whether or not currency is converted. The current Foreign Transaction fee is 2.50% of the transaction amount and will be displayed as a separate transaction on your statement.
Loss, Theft or Unauthorized Use
You are responsible for all authorized uses of your Card. Applicable law and our zero liability policy may protect you from liability for unauthorized purchases. You understand that your Card is not a credit card and is not protected by laws covering credit cards, such as the federal Truth in Lending Act.
Tell us at once if you believe the Card or PIN has been lost or stolen. Telephoning is the best way of minimizing your possible losses. If you believe that your card or PIN has been stolen, or that someone has accessed or may access money from your card without your permission, call us at 1-866-760-3156. If you fail to notify us promptly and you are grossly negligent or fraudulent in the handling of your card, you could lose all the money in the Card account.
Also, if your statement shows transactions that you did not make, tell us at once. If you do not tell us within sixty (60) days after the statement was made available to you online, you may not get back the money you lost after the sixty (60) days, if we can prove that we could have stopped someone from taking the money if you had told us in time. If you were grossly negligent or committed fraud in handling the Card, the amount of money you could lose for unauthorized purchases could increase to the maximum allowed by law.
If your Card has been reported lost or stolen, we will close your account to keep losses down.
Information about Your Right to Dispute Errors
In case of errors or questions about electronic transactions on the Card, call 1-866-760-3156 or write us at: Cardholder Services P.O. Box 550578 Ft. Lauderdale, FL 33355 as soon as you can, if you think the Card statement or receipt is wrong or if you need more information about a transaction listed on the statement or receipt. We must hear from you no later than sixty (60) days after the transaction in question has been made available to you on the online statement. If you tell us orally, we may require that you send your claim or question in writing within ten (10) business days.
The following information must be contained in that notice:
1. Your name and Card account number.
2. Describe the error or the transaction you are unsure about and explain as clearly as you can why you believe it is an error or why you need more information.
3. The dollar amount of the suspected error.
If you tell us orally, we require that you send us your complaint or question in writing within ten (10) business days.
Generally, we will tell you the results of our investigation within 10 business days after we hear from you and will correct any error promptly. If we need more time, however, we may take up to forty-five (45) calendar days to investigate your complaint or question. If we decide to do this, we will re-credit your Card account within ten (10) business days for the amount you think is in error, so that you will have use of the money during the time it takes us to complete our investigation. If we ask you to put your complaint or question in writing and we do not receive it within ten (10) business days, we may not re-credit your Card. If notice of an error involves an electronic fund transfer that was initiated other than in any state, territory or possession of the United States, the District of Columbia, the Commonwealth of Puerto Rico or any political subdivision of any of the above; or involves an electronic fund transfer resulting from a POS debit card transaction, the applicable time periods for action shall be ninety (90) calendar days in place of the forty five (45) calendar days.
If we decide that there was no error, we will send you a written explanation within three (3) business days after we finish our investigation and any credit made under the preceding paragraph must be repaid by you. You may ask for copies of documents that we used in our investigation.

Waiver Release from Third Parties
Unless the law provides otherwise, you waive and release us from any obligations that could arise due to defenses, rights and claims you have or may have against any third party on account of the use of the Card.
No Right to Stop Payment
You understand and agree that you are not permitted to stop payment on any purchase through the use of the Card.
We may at any time amend these terms and conditions. You will be notified of any change in the manner provided by applicable law prior to the effective date of the change. You specifically agree to accept such notice of change by notice sent to the last electronic mail address you have provided us. However, changes made for security purposes, can be implemented without prior notice. You can review the current Agreement on our website. If you notify us that either the Change in Terms and Conditions is not acceptable or that you no longer agree to accept to receive notices of Change in Terms and Conditions electronically, we may cancel or suspend this Agreement or any features or services of the Card at any time. We may cancel your right to use the Card at any time. However, your obligation under this Agreement and any charges made under it prior to cancellation will remain your responsibility. Any Card which we supply to you is our property and must be returned to us immediately upon demand or upon notice of cancellation or withdrawal of your Card. Any monies remaining on the Card account will be returned to you.
You may cancel this Agreement by returning the Card to us at the address above or calling member service at 1-866-760-3156 and closing your account. Any termination of this Agreement will not affect any of our rights or your obligations arising under this Agreement prior to termination. Upon receipt of your notice, the Card will be cancelled and any monies remaining on the Card account will be returned to you.
With our Zero Liability policy, your liability for promptly reported unauthorized transactions is $0. You will not be liable for any unauthorized purchases made through the Visa network of merchants, including those transacted on the Internet if you contact us within sixty (60) days after the posted transaction is made available to you on your online statement. This policy does not apply to ATM cash withdrawals or other PIN based transactions. You agree to assist us in determining the facts, circumstances and other pertinent information relating to any loss, theft or possible unauthorized use of your Card and to comply with such procedures as we may require in connection with our investigation. If you authorize someone to use your Card, you are responsible for all transactions they initiate using your Card until you notify us that such transfers are no longer authorized.
This Agreement and its performance shall be governed by the laws of the state of Virginia, United States of America, without regard to its conflict of laws provisions. You consent and submit to the exclusive jurisdiction of the state and federal courts located in the State of Virginia, United States of America, in all question and controversies arising out of your use of this Card product and this Agreement. Some states may not permit such consent, so the foregoing may not apply to you.
The Card and your obligations under this Agreement may not be assigned except to an authorized user who is an approved enrollee in the program. We may transfer our rights under this Agreement.
Use of the Card is subject to all applicable rules and customs of any clearing house or other association involved in transactions.
We do not give up our rights by delaying or failing to exercise them at anytime.
If any term of this Agreement is found by a court to be illegal or not enforceable, all other terms will still be in effect.
If we take legal action against you because of default in terms of this Agreement, you must pay reasonable attorney's fees and other costs of the proceedings. Your responsibility for fees and costs shall in no event exceed the maximum allowed by law.
